Hola Neighbor

The Hola Neighbor program addresses the social determinants of health exacerbated by the COVID-19 pandemic in the Hidden Valley neighborhood.

The Hola Neighbor program is a collaboration of five community organizations that provide support to residents who need it most.

Problem

A lack of a centralized system to track health screenings, referrals, and follow-ups, leading to inefficiencies in service coordination and reporting.

Solution

Build a case management system that could handle everything all while keeping communication clear between partners.

Results

Common Good Data worked with Hola Neighbor to build a data management system, design forms and surveys, and develop a regular process for reporting on program activities and outcomes. The results of this work can be highlighted in the 2024 Annual Report.

The Hola Neighbor program conducted a total of 433 health screenings leading to 705 referrals from Jan 2023 to June of 2024. 

98% of residents reported trusting Hola Neighbor and 92% were satisfied with their care. 

Hola Neighbor held 17 community health workshops and dozens of events in the community. 

Approximately 9 in 10 residents felt healthier, more socially connected, and felt they trusted more people in the neighborhood.
